Department of Human Services shall select at least 3,600 persons with intellectual or developmental disabilities from the PUNS database by June 30, 2024, with at least 600 selected by June 30, 2022, increasing to at least 1,200 annually by July 1, 2025.

Department shall pursue federal funding through Section 1115 demonstration waivers and Community First Choice Option (Section 1915(k)) to expand housing options and support services, with housing available in arrangements of 1-8 persons per unit.

Illinois Children and Adult Home and Community-Based Services Pilot Program shall be established and fully operational by June 30, 2025, funded by federal Community First Choice Option revenues to expand community-based services for eligible adults and children with disabilities.

By June 30, 2025, every individual selected from the PUNS list shall receive housing and support services unless choosing in-home services, with a flexible needs-based sliding scale to determine eligibility and placement.

Department shall submit annual progress reports to the General Assembly beginning May 31, 2023, with implementation contingent on receipt of all necessary federal waivers and approvals.
