Requires group and individual accident and health insurance policies and managed care plans amended, delivered, issued, or renewed after the act's effective date to cover A1C testing recommended by a health care provider for prediabetes, type 1 diabetes, and type 2 diabetes.

Defines A1C testing as blood sugar level testing used to diagnose and monitor diabetes and prediabetes, with coverage required for individuals meeting CDC-identified risk factors including age 35 and older, obesity, family history of diabetes, and certain racial and ethnic groups.

Requires insurance policies and managed care plans to cover vitamin D testing recommended by a health care provider for individuals with CDC-identified risk factors including osteoporosis, conditions affecting fat absorption, age 55 and older, darker skin color, obesity, and previous diagnosis of diabetes or kidney disease.

Extends A1C and vitamin D testing coverage requirements to the Illinois medical assistance program (Medicaid) by adding these provisions to the list of required health benefits.
