Amends the Civil Administrative Code to require the Department of Public Health's State Health Assessment (SHA) and State Health Improvement Plan (SHIP) to reduce health disparities and inequities, promote health equity, and conform to national Public Health Accreditation Board Standards.

Establishes a SHA and SHIP Partnership with a minimum of 15 members appointed by the Director of Public Health, including representatives from state agencies, local health departments, non-profits, healthcare providers, and communities experiencing health disparities.

Requires the SHA and SHIP Partnership to develop a community engagement process ensuring meaningful participation from individuals with lived experience in health disparities and inequities throughout the five-year implementation cycle.

Mandates the State Health Assessment include comprehensive data on population demographics, health status by race/ethnicity/gender identity/sexual orientation/disability status, social determinants of health, community priorities, and public health system strengths.

Requires the State Board of Health to submit annual reports by January 31 on SHIP implementation and community engagement activities to the Governor, General Assembly, and public, with consolidation into the new SHA/SHIP every five years.
