Creates a Voter Identification Card issued by the Secretary of State for registered voters without acceptable photo identification, provided free of charge and valid as long as voter maintains name and residence on the card.

Requires all voters to present either a government-issued photo identification card (such as driver's license, passport, or FOID card) or a Voter Identification Card to election judges before voting.

Establishes exemption allowing voters with religious objections to being photographed to vote a provisional ballot by signing an affidavit.

Applies photo identification requirement to provisional voting, early voting, and regular in-person voting, with 10-day window for provisional voters to provide required identification after election.
