Changes "may" to "shall" requiring health care providers, physician's designees, and pharmacist's designees to provide immunization data to the state registry unless a patient opts out via written exemption form.

Requires written information and exemption forms to state that health care providers "shall" (not "may") report immunization data to the Department of Public Health for entry into the registry.

Mandates that information materials include a description of the registry, notification of the provider's reporting requirement, patients' right to exemption, and instructions for removal of information from the registry.

Makes the immunization data registry opt-out rather than opt-in by establishing immunization reporting as mandatory unless a patient or parent/guardian completes and files an exemption form.

Takes effect January 1, 2023.
