• Amends the School Code to define student sex in interscholastic athletic programs as biological gender assigned at birth, based on the student's official birth certificate filed at or near time of birth.

• Requires high schools to only participate in post-season basketball tournaments conducted by organizations that offer identically structured tournaments for both boys and girls with equal numbers of teams.

• Adds new section to Board of Higher Education Act requiring public universities to designate intercollegiate athletic teams based on biological sex at birth (males/men/boys, females/women/girls, or coed).

• Prohibits athletic teams designated for females, women, or girls from being open to students of male sex, while allowing teams designated for males, men, or boys to be open to students of female sex.

• Applies identical athletic team designation requirements to public community colleges under the Public Community College Act.
