Restricts pre-election notice of permanent vote by mail status to qualified voters who have not already enrolled in permanent vote by mail and have not opted out of receiving such notices, rather than notifying all qualified voters.

Changes the notification timeframe from "before a general or consolidated election" to specifically "not more than 90 days nor less than 45 days before a general election."

Adds an opt-out option to the permanent vote by mail application allowing voters to decline future notices while retaining the ability to apply for vote by mail status independently.

Requires the application notice to inform voters they may mark the permanent vote by mail option or opt out of receiving future permanent vote by mail notices on the attached application.

Modifies the permanent vote by mail application form to include the opt-out option and clarifies that voters selecting opt-out need not provide a mailing address.
