Creates the Illinois Health Services Program providing universal health coverage to all state residents, covering primary care, specialty care, inpatient/outpatient services, emergency care, prescription drugs, mental health, dental, vision, long-term care, and substance abuse treatment with no deductibles or copayments

Prohibits private health insurers from selling coverage that duplicates program benefits and bans investor-ownership of health delivery facilities including hospitals, HMOs, nursing homes, and clinics, with compensation provided to current investor-owners

Establishes the Illinois Health Services Trust funded by 20% individual contributions, 20% business contributions, and 60% government funding, plus federal Medicaid/Medicare funds, with administrative costs capped at 12% initially and decreasing to 5% after four years

Pays hospitals and health facilities through negotiated annual global budgets for operating expenses, while physicians can choose fee-for-service, salary, or capitation payment models; requires conversion of for-profit HMOs and group practices to nonprofit status

Creates a Pharmaceutical and Durable Medical Goods Committee to negotiate drug and medical supply prices through competitive bidding, and requires implementation of statewide electronic medical records within 7 years of the effective date of July 1, 2023
