Designates a statewide association representing centers for independent living as the lead agency to administer the program, distributing funds to Illinois's 22 centers for independent living covering all 102 counties

Guarantees each center for independent living access to at least $105,000 annually for home modification projects, with excess funds subject to reallocation during the 4th fiscal quarter

Eligibility requires applicants to be Illinois residents with a disability, have income not exceeding 200% of area median income, and be homeowners or tenants with landlord permission for modifications

Establishes a Home Modification Subcommittee to review assessment reports for new centers' first 3 requests and any requests exceeding $35,000, with contractors required to be selected through competitive bidding from at least 2 licensed bidders
