Amends the Unified Code of Corrections to require the Department of Corrections and Department of Juvenile Justice to collect and maintain ethnic, racial, and residential address data on incarcerated individuals in accordance with the No Representation Without Population Act.

Modifies Section 3-5-1 to include a committed person's last known complete street address prior to incarceration or legal residence in their master record file.

Updates Section 5-4-1 to require transmission of demographic information (race, Hispanic/Latino origin, age, and last known address) to the Department of Corrections when a defendant is sentenced.

Amends the No Representation Without Population Act Section 2-10 to clarify that the Department must report incarcerated persons' last known pre-incarceration addresses or legal residences to the State Board of Elections, using alternative addresses from parole or release programs if the primary address is unknown.

Maintains confidentiality protections by prohibiting personal identification from demographic data shared with the State Board of Elections, limiting use to aggregated redistricting data by census block.
