Amends Section 12-4.4a of the Criminal Code to clarify that criminal neglect of long-term care facility residents applies to "a resident's" life and health rather than "an elderly person's or person with a disability's" life and health.

Establishes that abuse of long-term care facility residents is a Class 3 felony, criminal neglect is a Class 4 felony (or Class 3 if resulting in death), and simple neglect is a petty offense.

Sets criminal penalties for caregiver abuse or neglect of elderly persons or persons with disabilities as a Class 3 felony, or Class 2 felony if resulting in death, with a minimum 3-year and maximum 14-year prison term.

Amends Section 17-56 regarding financial exploitation of elderly persons and persons with disabilities by changing "over 70 years of age" to "70 years of age or older" and adds civil liability provisions allowing treble damages, attorney fees, and court costs for victims.

Authorizes prosecutors to petition courts to freeze assets of defendants charged with financial exploitation involving property valued over $5,000 for purposes of victim restitution.
