IL HB2530

Establishes a 5-year Managed Primary Care Demonstration Project under the Department of Healthcare and Family Services to serve underserved communities lacking accessible health and medical services.

Provides supplemental preventive care services to medical assistance recipients, including those in the state's managed care program.

Requires the Department to contract with a health care organization to deliver nine specific services: patient navigators, preventive health care plans, home health consultants, telehealth consultants, patient portals, virtual health hubs, community health centers, mobile health stations and kiosks, and call centers.

Must be implemented no later than 6 months after the Act becomes effective.

Takes effect immediately upon becoming law.
