Requires the Illinois Environmental Protection Agency to contract with an existing multistate chemical data collection entity to implement a publicly accessible data collection interface by January 1, 2026.

Mandates manufacturers of PFAS or products containing intentionally added PFAS sold, distributed, or imported into Illinois to register on the interface by July 1, 2026, and annually thereafter with specified product and chemical information.

Required registration data includes product name and type, universal product code, purpose of PFAS use, identity and amount of PFAS compounds, quantity of products sold or imported, and manufacturer contact information.

Authorizes the Agency to adopt rules, provide technical assistance to manufacturers, and clarify reporting requirements to prevent duplication among reporting entities.

Exempts products regulated by the Food and Drug Administration as drugs, medical devices, dietary supplements, and veterinary products from registration requirements; violations subject to civil penalties under Section 35.
