Amends the School Code to require individualized education plans (IEPs) to include measurable post-secondary goals and transition services beginning no later than when a student turns age 14½ (or younger if appropriate), updated annually.

Mandates school districts provide students and parents with information about career and technical education (CTE) opportunities, including programming options, scope and sequence of study, and locations of those programs.

Requires school districts to inform students and parents about dual credit courses available under the Dual Credit Quality Act, including entry criteria and information about courses available for dual credit or high school credit only.

Expands transition planning to include potential participants from state agencies, community colleges, adult service providers, and disability services coordinators, with the IEP identifying responsible parties for coordinating and delivering transition services.

Requires school districts to follow up on referrals to outside entities, ensure students are successfully linked to services, monitor progress toward transition goals, and establish specific review periods during the school year to assess services provided.
