Secretary of State shall issue standard Illinois Identification Cards to persons released from Department of Corrections or Department of Juvenile Justice upon receipt of birth certificate, social security card, photograph, proof of residency, and application transferred via secure method by the respective department.

Secretary of State shall issue limited-term Illinois Identification Cards valid for 90 days to released persons unable to present birth certificate and social security card but who provide a department verification form confirming date of birth, social security number, and Illinois residence address.

Department of Corrections must begin obtaining certified copies of birth certificates and duplicate social security cards for committed persons within 45 days of their receipt if the person lacks access to those items.

Department of Corrections shall transmit identification card applications to Secretary of State 60 days before scheduled discharge or upon receipt of person's birth certificate and social security card, whichever occurs later.

Department must provide discharged persons information about obtaining standard or limited-term identification cards if they have not already been issued one at an earlier stage.
