Secretary of State shall issue Voter Identification Cards to registered voters who lack acceptable photo identification, with cards including voter's name, signature, photograph, state seal, and current residence address.

Defines "acceptable photo identification" as Illinois Driver's License, state ID card, disabled person ID card, firearm owners ID card, U.S. passport with current address, or any government-issued ID with voter's name, current photograph, and current address.

Requires voters to present acceptable photo identification or a Voter Identification Card to election judges for identity verification when voting in person, by early ballot, or by mail.

Provides exemptions allowing indigent voters (income at or below 125% of federal poverty guidelines) and voters with religious objections to being photographed to vote provisional ballots with signed affidavits instead of providing identification.

Modifies provisional ballot procedures to add a new category allowing voters who fail to provide acceptable photo identification or Voter Identification Card to cast provisional ballots, with 10 days to provide the required identification for ballot validity.
