• Creates the Reporting of Deaths in Custody Act requiring law enforcement agencies to report all in-custody deaths to the Illinois Criminal Justice Information Authority within 30 days, including cause of death, demographic information, and emergency care details.

• Designates reports as public records under the Freedom of Information Act and requires the Authority to publish individual reports quarterly and issue an annual public report with trends, causes of death, and recommendations to reduce in-custody deaths.

• Requires law enforcement to notify deceased's family or emergency contact with accurate factual information about the death, appoint a dedicated family liaison officer, and disclose medical records to the family upon request.

• Amends the Illinois Identification Card Act to require the Secretary of State to issue annual reports tracking permanent identification cards issued to persons released from Department of Juvenile Justice and Department of Corrections using verification forms.

• Amends the Unified Code of Corrections to require Department of Corrections and Department of Juvenile Justice to track and annually report on birth certificates, social security cards, and verification forms obtained for released persons, and to transmit in-custody death reports to their respective departments.
