Requires the Department of Healthcare and Family Services, in collaboration with the Department of Human Services' Division of Mental Health, to allow hospital presumptive eligibility for medical assistance for individuals in psychiatric crisis presenting in emergency rooms who meet federal criteria.

Mandates development of a referral process, with meaningful stakeholder input, to connect individuals to community-based mental health providers or appropriate organizations for medical assistance enrollment immediately following hospital or emergency room discharge.

Requires referral for community-based treatment services following hospital or emergency room discharge.

Directs the Department to amend its Illinois Continuity of Care and Administrative Simplification 1115 Waiver within 60 days of the act's effective date if amendments are necessary to implement the hospital presumptive eligibility requirement.
