Requires Illinois State Police to include admission dates and facility name in notices when denying or revoking a Firearm Owner's Identification Card (FOID) based on mental health facility history.

Establishes that if the FOID Review Board fails to render a decision within 90 days of an appeal filing, the failure constitutes rejection of the appeal and applicant may appeal to circuit court for relief.

Sets the replacement cost for a combined FOID and concealed carry license at $5 when the person has changed their address.

Amends the Firearm Concealed Carry Act to make conforming changes related to fee provisions.
