Amends the Illinois Public Aid Code to create a per-claim add-on payment of $95 per treatment for renal dialysis services provided in skilled nursing facilities beginning January 1, 2025.

Defines "certified home dialysis provider" as an end stage renal disease facility that provides dialysis treatment or training and is approved by the federal Centers for Medicare and Medicaid Services to provide dialysis home training support services.

Establishes that on-site home dialysis in nursing facilities benefits residents by reducing off-site travel and associated healthcare risks while creating care opportunities for hospital patients requiring post-acute dialysis services.

Recognizes that on-site home dialysis is costlier for providers than conventional outpatient dialysis due to higher labor costs per treatment and greater medication and staff involvement for higher-acuity patients.
