Requires owners or operators of community water systems, community wastewater systems, and nontransient, noncommunity water systems to conduct PFAS monitoring by December 31, 2025 using EPA-established laboratory methods.

Mandates continued quarterly monitoring if initial testing detects PFAS contaminants at or above 20 nanograms per liter until levels drop below that threshold.

Requires the Illinois Environmental Protection Agency to direct water system operators to issue public notices if PFAS is detected at or above 20 nanograms per liter, informing users of detected concentrations and potential health risks.

Extends civil penalties to water system owners or operators who violate the Act, with first violations subject to penalties up to $5,000 and subsequent violations up to $10,000, with revenues deposited to the Environmental Protection Trust Fund.

Adds definitions for "Board," "community wastewater system," "community water system," "nontransient, noncommunity water system," and "public water system" to the PFAS Reduction Act.
