• Requires substance abuse programs operating in Illinois to notify the personal representative of a patient within 24 hours (verbally) and 5 days (in writing) after the patient's death.

• Requires mental health and developmental disabilities facilities operating in Illinois to notify the personal representative of a service recipient within 24 hours (verbally) and 5 days (in writing) after the recipient's death.

• Defines "personal representative" according to 45 CFR 164.502(g) and requires notices to comply with federal regulations 42 CFR 2.15(b) and 45 CFR 164.502(g).

• Becomes effective immediately upon enactment.
