Removes the 4-prescription drug limit and prior authorization requirement that prohibited reimbursement for more than 4 prescriptions (including 3 brand name drugs) per 30-day period under the medical assistance program.

Eliminates exemptions that previously excluded immunosuppressant drugs, oncology drugs, anti-retroviral drugs, and antipsychotic drugs from the 4-prescription limit.

Maintains existing provisions for utilization controls on narcotic medications, specialty drugs, HIV/AIDS treatments, and psychotropic medications used as chemical restraints in nursing homes.

Preserves requirements for pharmacy reimbursement rates, prior approval for certain drug classes, and medication therapy management programs.

Requires providers eligible for the federal Drug Pricing Program under Section 340B to enroll and not exclude fee-for-service Medicaid from participation.
