Amends the Illinois Public Aid Code Section 5-2.06 regarding reimbursement rates for Children's Community-Based Health Care Centers effective January 1, 2025.

Increases the Department reimbursement rate from $950 to $1,650 per diem for eligible individuals receiving nursing care services at these centers.

Reimburses at the lower of the center's usual and customary charge to the public or the Department rate of $1,650.

Applies to services provided for transitioning children from hospitals to home placement or other appropriate settings and reuniting families, with a maximum duration of 120 days.

Exempts payments under this rate from the 2.7% rate reduction required under Section 5-5e of the Illinois Public Aid Code.
