Requires the Department of Healthcare and Family Services to maintain a rate add-on of at least $6.15 per day for providing 2 meals daily in supportive living program homes and community-based service waivers, as approved by the Centers for Medicare and Medicaid Services starting January 1, 2023.

Establishes specific Medicaid rate requirements for supportive living facilities, including a 54.3% minimum of average nursing facility per diem rates beginning July 1, 2019, with updates tied to nursing facility rate changes.

Beginning January 1, 2024, requires dementia care rates for supportive living services to be no less than 1.5 times the non-dementia care rate, subject to federal approval.

Authorizes a $26 per diem supplemental payment to supportive living facilities for 12 months (April 1, 2021 through March 31, 2022) using enhanced federal medical assistance funds under the American Rescue Plan Act of 2021.

Restricts expansion of supportive living dementia care settings approved after January 1, 2024 to sites that meet distance requirements (4 miles in Cook County, 12 miles in collar counties) and limits new non-dementia units to a 1:1 ratio with dementia units.
