Amends the Psychiatry Practice Incentive Act to establish a Collaborative Care Demonstration Grant program administered by the Department of Public Health to support primary health care practices implementing the psychiatric Collaborative Care Model.

Grants awarded under the program must be a minimum of $100,000, increasing by $1,000 per 1% share of patients expected to be enrolled in Medicaid, up to a maximum of $500,000 per award.

Grants may be used for hiring staff, establishing contracts with psychiatric consultants and behavioral health managers, purchasing software for patient registries and measurement-based care, and other necessary purposes.

Director of Public Health may contract with collaborative care technical assistance centers to provide training and support to primary care practices on implementing the model, including financial modeling, staffing development, electronic health records integration, and staff training.

Program becomes effective July 1, 2024, and is subject to appropriation; annual reports to the General Assembly must include information on integration of behavioral health care into primary care settings in designated shortage areas.
