Repeals provisions of the Public Utilities Act requiring the Illinois Commerce Commission to establish a Universal Telephone Service Assistance Program for low-income residential customers and to adopt enhanced enrollment rules for lifeline service.

Repeals requirements that Electing Providers continue offering optional telephone service packages to existing and new customers through July 1, 2017, allowing them to discontinue these packages after that date with proper notice.

Amends the Broadband Adoption Fund Act to define "broadband internet" as a minimum service level of at least 25 megabits per second download speed and expands "provider" to include communication services providers.

Requires telecommunications carriers, wireless carriers, cable providers, and VoIP providers to notify customers of the option to contribute monthly fixed amounts ($1, $2, or $5) to the Illinois Broadband Adoption Fund through their bills, with the right to cease contributions at any time.

Authorizes the Department of Human Services to provide vouchers for eligible households to purchase broadband internet service and devices used to connect to the internet, including computers, modems, and routers.
