Amends the Vital Records Act to require registration of births resulting in stillbirth occurring after at least 20 completed weeks of gestation within 7 days with the local registrar of the district where the birth occurred.

Establishes procedures for certificate preparation: in institutions, the person in charge must obtain required information and the attending physician must verify medical information within 24 hours; outside institutions, certificate shall be prepared by the attending physician, another person in attendance, or the parents/person in charge of premises in order of priority.

Reduces the gestation period threshold for stillbirth certificate registration from 26 weeks to 20 weeks.

Requires the preparer to inform the woman who delivered the stillborn child of the right to decline the certificate, with declination only preventing preparation if requested in writing on a State Registrar-prescribed form.

Allows parents to leave blank any references to the stillborn's name on the certificate if they choose not to provide one.
