Establishes pilot programs in Lee, Whiteside, Ogle, and Tazewell counties allowing courts to order involuntary treatment for adults 18+ with substance use disorders, effective January 1, 2025 through January 1, 2029.

Requires court-ordered involuntary treatment only when a person has a substance use disorder, presents imminent danger to self/family/others or substantial likelihood thereof, and can reasonably benefit from treatment.

Sets procedures for filing petitions by spouses, relatives, or legal guardians; requires court examination of petitioner under oath and medical examinations by physician and psychiatrist/psychologist before hearings.

Authorizes courts to order treatment periods of either 60 or 360 consecutive days; permits emergency 72-hour hospitalization when clear and convincing evidence shows imminent danger.

Requires Department of Human Services to submit effectiveness and efficiency report to General Assembly by July 1, 2029, including data on persons treated, treatment services provided, and most effective services used.
