Expands the "Student Permanent Record" definition to include reports of psychological evaluations (intelligence and personality information), academic information from test administration or interviews, achievement-level test results, special education records, and Section 504 Rehabilitation Act records.

Removes intelligence test scores, aptitude test scores, psychological and personality test results, teacher evaluations, and other information of clear relevance to education from the list of items that may be included in Student Permanent Records.

Requires schools to provide copies of student school records at no cost to the Guardianship and Advocacy Commission, independent service coordination agencies, or Equip for Equality when consent is provided by the student or parent/guardian to establish access to adult disability services and benefits.
