• Amends the Illinois Act on the Aging to require long-term care facilities, supportive living facilities, assisted living establishments, shared housing establishments, and State-operated developmental centers to display the Long Term Care Ombudsman Program's statewide toll-free telephone number, address, and website in multiple conspicuous public places accessible to residents and visitors.

• Requires each facility to post the Ombudsman Program's statewide toll-free telephone number and a link to its website on the facility's home page, with exceptions for facilities without unique websites or parent companies operating in multiple states.

• Establishes a Consumer Choice Information Report system to be created by the Office in collaboration with the Attorney General, requiring licensed long-term care facilities to complete forms providing information on medical care, staffing, safety, meals, rooms, family visitation, and other facility details.

• Makes Consumer Choice Information Reports accessible to the public through the Office's website and the Department of Human Services website for facilities under the ID/DD Community Care Act or MC/DD Act.
