Insurance policies amended, delivered, issued, or renewed in Illinois after January 1, 2026 must cover home test kits for sexually transmitted infections (STIs), including HIV, when ordered by a clinician, with no cost-sharing requirements except where prohibited by health savings account rules.

Medicaid (medical assistance program) must provide coverage for home STI test kits and laboratory processing costs consistent with requirements for commercial health plans, effective January 1, 2026.

Creates the Illinois AIDS Drug Assistance Program (Illinois ADAP) establishing "Rapid Start for HIV Treatment" pilot sites in each HIV Care Connect Region to initiate antiretroviral therapy within 7 days of diagnosis, with conditional approval pathway allowing enrollment within one business day.

Pilot sites must operate under five principles: "Nothing About Us Without Us" (community involvement), Equity, Self-Determination, Reduce Stigma, and Safe Spaces, with comprehensive reporting on enrollment, outcomes, and demographic data.

Requires county jails to provide prisoners with written HIV/AIDS information during medical intake and offer free HIV testing options, with Cook County required to implement opt-out testing procedures and the Department of Corrections to report compliance in annual inspection reports.
