Amends the Reproductive Health Act by adding Section 1-23 to require health care professionals to provide specified information before performing an abortion.

Requires the Department of Public Health to develop information that must be provided to patients, including a comprehensive list of geographically available public and private agencies offering family planning services, adoption services, natural family planning information, and ultrasound imaging services with contact details.

Mandates disclosure of availability of governmentally funded programs serving pregnant patients and children, including medical services for the pregnant person.

Requires patients to attest in writing on a Department-prescribed form that they received the required information before an abortion can be performed.
