Amends the Illinois Health Facilities Planning Act to require nursing homes operated by a county to file a Safety Net Impact Statement with applications for substantive projects, service discontinuation, or change of ownership.

Expands the definition of "safety net service providers" to include nursing homes operated by a county, requiring them to report charity care and Medicaid patient data for the 3 fiscal years prior to application.

Requires the State Board Staff Report to include findings about an application's safety net impact when it involves a substantive project, ownership change, or service discontinuation at a county-operated nursing home, and directs these findings to be considered in determining public interest compliance.

Modifies the Counties Code to remove the requirement that a 2/3 board majority vote is needed to sell, dispose of, or lease county nursing home properties, and establishes specific referendum ballot forms for county voters to approve the sale, closure, or lease of county nursing homes.

Requires county nursing homes proposing a change of ownership to disclose the number of Medicaid and Medicare certified beds for the 3 fiscal years prior to the application.
