Amends the Managed Care Reform and Patient Rights Act to establish requirements for carriers offering provider panels, including filing notice of panel development with the Department of Public Health prior to establishment.

Requires carriers to notify enrollees at least 60 days before terminating a health care provider from the panel and to inform enrollees of their right to continue receiving services from the terminated provider for at least 90 days, except for cause-based terminations.

Prohibits carriers from denying provider participation based on gender, race, age, sexual orientation, gender identity, religion, or national origin, and prohibits contracts requiring providers to indemnify carriers for carrier negligence or breach of contract.

Mandates that carriers providing preauthorization for medical treatment maintain personnel available at all times when preauthorization is required, and prohibits contract provisions requiring providers to deny medically necessary and appropriate covered services.

Requires carriers to provide group policyholders written notice of benefit reductions at least 60 days in advance and to make provider panel lists available to enrollees at least annually, updated monthly if provided electronically.
