Extends infertility coverage mandate for group health insurance policies providing pregnancy-related benefits to all policies (not just those covering 25+ employees) effective January 1, 2026, and expands required procedures to include surgical sperm extraction and preimplantation genetic testing for aneuploidy, chromosome structural rearrangements, and monogenic disorders.

Adds annual menopause health visit coverage requirement for individuals 45 years and older in group and individual health insurance policies with more than 25 employees, effective January 1, 2026, with no cost-sharing unless it would disqualify a high-deductible health plan from health savings account eligibility.

Applies new infertility and menopause coverage requirements to State Employees Group Insurance Program, county self-insured health plans, municipal self-insured health plans, school district employee insurance, Health Maintenance Organizations, Limited Health Service Organizations, and Voluntary Health Services Plans.

Creates temporary infertility coverage for State Employees Group Insurance Program from January 1, 2024 through June 30, 2026, including in vitro fertilization, embryo transfer, artificial insemination, and preimplantation genetic screening at facilities meeting specified clinical standards.

Updates Illinois Insurance Code Section 356m to modify infertility coverage requirements and adds references to new insurance coverage mandates across multiple state employee and health plan regulatory provisions.
