Mandates that notifications include all contract terms, policy manuals, fee schedules, and an opt-in form requiring providers to affirmatively choose to join the new insurer's network.

Prohibits insurers from canceling or penalizing network dentists based on whether they accept the terms of a network assignment or lease.

Requires insurers to offer providers the option to contract directly with the entity gaining access to the network before accepting assignment or lease terms.
