Restricts election authority notices about permanent vote by mail status to 45-90 days before general elections only (removes consolidated elections from requirement) and excludes voters already enrolled in permanent vote by mail status or those who have opted out.

Adds an opt-out option to the permanent vote by mail application allowing registered voters to decline receiving future notices or applications about permanent vote by mail status.

Permits voters who opt out to still apply individually for single-election vote by mail ballots or permanent vote by mail status by submitting applications directly to their election authority.

Updates the permanent vote by mail application form to include the opt-out option with language clarifying that opting out does not prevent future independent applications for vote by mail services.

Requires the address field on permanent vote by mail applications to be left blank if a voter selects the opt-out option.
