Establishes the Illinois Farm to Food Bank Program within the Department of Human Services to expand access to locally grown, raised, or processed foods through the emergency food system.

Program shall acquire and distribute agricultural products (fruits, vegetables, meat, poultry, dairy, eggs) from Illinois sources to food banks, and provide capacity-building grants for storage, transportation, and distribution infrastructure.

Requires dedicating at least 75% of available funds to food acquisition and distribution, with program participants matching Farm to Food Bank resources using non-state funds.

Creates a Farm to Food Bank Advisory Council with representatives from food banks, agriculture, Department of Human Services, Department of Agriculture, and academic institutions to facilitate partnerships and provide oversight.

Department of Human Services shall engage a not-for-profit entity with statewide reach representing multiple food banks to administer the program and must adopt implementing rules including health/safety standards, food quality requirements, and equity advancement goals.
