Requires group health insurance policies covering more than 25 employees with pregnancy benefits to include infertility treatment coverage effective January 1, 2024, including in vitro fertilization and procedures to screen fertilized eggs before implantation.

Mandates coverage for annual menopause health visits for individuals age 45 and older without cost-sharing, effective January 1, 2024, for group and individual policies with more than 25 employees.

Requires coverage for all injectable medicines prescribed on-label or off-label to improve glucose control or weight loss for adults with prediabetes, gestational diabetes, or obesity, effective January 1, 2024.

Provides religious institution exemption allowing policies issued by religious organizations to exclude infertility coverage that violates their religious and moral teachings.

Applies requirements to state employees, counties, municipalities, schools, health maintenance organizations, limited health service organizations, and medical assistance programs.
