Amends the Illinois Public Aid Code to change the effective date for reimbursement of Children's Community-Based Health Care Centers from January 1, 2020 to January 1, 2025.

Increases the Department of Healthcare and Family Services reimbursement rate for Children's Community-Based Health Care Centers from $950 to $1500 per diem.

Maintains reimbursement at the lower of the center's usual and customary charge to the public or the Department rate of $1500.

Applies to nursing care services for transitioning children from hospital to home placement or other appropriate settings for a maximum of 120 days.

Exempts these payments from the 2.7% rate reduction required under Section 5-5e of the Illinois Public Aid Code.
