• Amends the Assisted Living and Shared Housing Act to include diagnosis of Alzheimer's disease or related dementia as a significant change requiring resident reassessment.

• Requires service plans to be developed with the resident and resident's representative (if any), and mandates residents receive copies of assessments and service plans upon request.

• Adds protections for residents following hospitalization, medical leave, or absence by prohibiting facilities from refusing readmission if care needs do not exceed the facility's licensed services, and requires Department orders for readmission to be immediately implemented.

• Establishes new enforcement provisions including fines for facilities failing to readmit residents ordered by the Department and requirements to submit correction plans within 30 days.

• Amends the Nursing Home Care Act to increase minimum notice period for involuntary transfer or discharge from 21 to 30 days and adds similar readmission protections and enforcement mechanisms for nursing home facilities.
