By January 1, 2025, the Department on Aging must seek federal approval from Centers for Medicare and Medicaid Services for waivers or State Plan amendments to provide monthly monitoring payments to care coordination units for each active eligible participant in the Community Care Program who is receiving services but has not utilized authorized services in the preceding month.

Managed care organizations must reimburse care coordination units the full monthly monitoring payment for services provided in accordance with the Act.

Care coordination units must provide documentation demonstrating contact attempts with individuals to confirm they no longer need services in order to receive administrative payments.

"Active participant" is defined as a person age 60 or older who has been found eligible to receive Community Care Program services.

The Department must secure federal financial participation for expenditures made under this provision for State Fiscal Year 2025 and every subsequent fiscal year.
