Requires survivors to provide a sworn statement or affidavit attesting to domestic violence, along with supporting documentation such as a police report, protective order, or shelter documentation.

Mandates health care practitioners inform survivors of the opt-out option and submit bills to the Department of Human Services for reimbursement through the Domestic Violence Prevention and Intervention Program if the survivor chooses not to bill their insurer.

Permits reimbursement regardless of when the domestic violence incident occurred, limited to medical forensic services directly related to the incident.
