Requires the Illinois Department of Public Health to approve training resources that support informed decisions and accurate completion of the Uniform POLST (Practitioner Orders for Life-Sustaining Treatment) form

Mandates the Department post approved POLST training materials on its website

Establishes yearly training availability for personnel at nursing homes licensed under the Nursing Home Care Act and other health care providers that use the Uniform POLST form

Adds the Uniform POLST form to the list of advance directive documents that must be published in Spanish, along with the Living Will Declaration, Power of Attorney for Health Care, and Declaration of Mental Health Treatment forms
