Creates special "EMS chief" license plates beginning with the 2027 registration year, available to emergency medical services chiefs of municipalities or fire protection districts upon request and payment of applicable fees

Plates may be affixed to passenger vehicles or motor vehicles weighing not more than 8,000 pounds owned by an EMS chief, municipality, or fire protection district

Authorizes vehicles with EMS chief plates to use red/white oscillating, rotating, or flashing lights and blue lights when responding to emergencies, and permits sirens, whistles, or bells on EMS chief vehicles

Requires EMS chiefs who resign, are terminated, or reassigned to a different position to immediately surrender their license plates to the Secretary of State

Establishes permanent vehicle registration plates for municipality or fire protection district-owned EMS chief vehicles for a one-time $8.00 fee beginning in 2027
