Creates the Health Care Transparency Act requiring the Department of Public Health to identify reproductive health care services, LGBTQ health care services, and end-of-life care services that may be denied for nonmedical reasons in Illinois

Requires the Department to develop a disclosure form by January 1, 2026, showing which identified services are available, restricted, or subject to referral limitations at each covered entity (hospitals, ambulatory surgical centers, and freestanding emergency centers)

Mandates covered entities submit completed disclosure forms within 60 days of issuance and provide updated forms to the Department within 30 days of any service availability changes

Beginning March 1, 2026, covered entities must provide disclosure forms to patients as part of the informed consent process before health care services begin and maintain records of patient receipt

Requires the Department to publish all disclosure forms on its public website, adopt rules for investigating complaints and assessing fines, and develop a public education program on how denial of care impacts health care access and vulnerable communities
