Requirements apply across multiple types of coverage including state employee group insurance, county and municipal self-insured plans, school district employee insurance, Health Maintenance Organizations, Limited Health Service Organizations, Voluntary Health Services Plans, and Medicaid

The Department of Insurance is responsible for enforcing the new cost-sharing limitations
