Amends the Animal Welfare Act to require a licensed veterinarian to examine every dog or cat imported into Illinois from another state by an animal shelter or animal control facility

Animal shelters and animal control facilities must receive an examination document from the veterinarian for each imported animal

Shelters and control facilities must provide the veterinary examination document to adopters prior to adoption for any dogs or cats imported from out of state

Adds the veterinary examination document requirement to the existing list of disclosures that shelters must provide in writing and post near animal cages

Disclosure records must be maintained by shelters for 2 years from the date of adoption
