Amends the Animal Welfare Act to require animal shelters and animal control facilities to disclose additional information when offering dogs for adoption

Mandates that shelters provide the name, address, and phone number of the organization from which an adopted dog was originally obtained

New disclosure requirement applies specifically to dogs, not cats, and must be provided in writing to adopters prior to adoption

Information must also be posted on or near the dog's cage and included in the signed acknowledgement of disclosures form that both the adopter and facility representative must sign
